Ageing well in the west

Your opportunity to help shape health services for older people experiencing 'mild frailty' in the west of Haringey.

'Ageing Well in the west' is for people living with ‘mild frailty’* who live in the west of the borough. Its aim is to better know and support people earlier who may be beginning to struggle with daily living, such as getting around and about.

The project is run by NHS North Central London Integrated Care Board (NHS NCL ICB). It seeks to improve how people connect to local solutions they might value, and to ensure the right services and support are there to help meet their needs.

The project is focussed on the west of the Borough, including GP practices, as there is a greater proportion of older patients and residents in that part of Haringey. Other partners in the project include NHS Community Health and  hospitals, Haringey Council, and the Voluntary and Community Sector.

* There is already support offer in place for those with more significant frailty, via the Multi-Agency Care and Coordination Team. This is a service available to people throughout Haringey and can be accessed through your GP.

We are working in partnership with Haringey residents and patients living in the Borough to design the offer. So far, the project has been very much shaped by the feedback we have received from people who live locally. If our approach is successful, it will both form a part of wider plans to support people of all ages in the west, and we will integrate many features of our approach to support older people living in east and central Haringey.
